This webinar will explore the vision for a better use of NHS facilities to improve community health and resilience. Best-practice examples will be discussed during the session, not only focusing on the successes but also the challenges that can be encountered in the co-location of primary care services with community services and social prescribing.

This webinar will focus on existing NHS estate, and a follow-up session will be held in the coming months to concentrate on building new health and well-being hubs.
